When you use Pathway Health’s services as a patient to request and order medicinal cannabis from licensed producers, you will be asked to provide Pathway Health with certain personal information about you such as your first name, last name, address, phone number, email address and residence, as well as your health card and credit card number. With your consent, we will also ask you to provide us with certain personal health information, such as the name and location of your doctor, health documentation, your prescribed daily amount of medicinal cannabis, the period of authorized use and the quantity of medicinal cannabis you wish to purchase. We may also ask you to provide your emergency contact information.

Information Pathway Health Obtains from Referral Sources

You may be referred to Pathway Health by your health care provider, such as your medical clinic, pharmacy or health care professional. With your consent, your health care provider may transfer your medical records and health documentation to Pathway Health so we can provide you with our services.

Information Automatically Gathered

Pathway Health uses cookies to automatically collect certain information, including personal information, when you visit our website. For example, Pathway Health may collect and store information related to your use of our website, including pages visited, duration at the site, the date and time of your visit. The information collected is used to help Pathway Health provide a more relevant, efficient and useful site experience for you.         • Licensed Producers – Pathway Health will communicate your personal information with Licensed Producers in order to facilitate your registration so they can fulfill your order.
        • Health Care Practitioners – with your express consent, Pathway Health may share your personal information with your health care practitioner in connection with your use of medicinal cannabis.

Public Profiles. If you provide personal information to the Pathway Health Blog, this information will be publicly available and therefore may be seen by other users.

Trusted Third Party Service Providers. Pathway Health may use third parties to provide technical support and perform tracking and reporting functions, conduct quality assurance testing, and provide other services in connection with its services. Pathway Health may share your personal information with them so that they can perform the required services.

Third Parties. Upon receipt of a written request by a client or patient to share personal information with a third party, Pathway Health will provide the requested data.

Business Transfers. Pathway Health is committed to protecting all information provided to us by patients by maintaining physical, technical and procedural safeguards that are appropriate to the sensitivity of the personal information. While Pathway Health cannot guarantee absolute security, the Company has implemented and reviews security protocols to protect patient information. Access to patient information is limited & restricted.

How Long We Keep Your Information

Pathway Health retains your personal information for the period of time that you are actively accessing our services. Pathway Health may however retain personal information in accordance with Pathway Health’s data retention policies, under which we store relevant information to comply with laws, College of physicians and Surgeons medical record keeping guidelines and to prevent fraud, resolve disputes, troubleshoot problems, assist with any investigations and take other actions otherwise permitted by law.

Any external websites linked from the Pathway Health website do not operate under this Privacy Policy. Pathway Health strongly recommends that patients examine the privacy policies of third party websites regarding their procedures for collecting, storing and using personal information.
